Topiramate: May decrease the serum concentration of Contraceptives (Progestins).
Management
Caution patients that this combination may be associated with reduced contraceptive effectiveness. Consider adding an additional (non-hormonal) contraceptive method.

Mechanism
Topiramate: May decrease the serum concentration of Contraceptives (Estrogens). Contraceptive failure is possible.
Management
Risk appears greatest for higher topiramate doses (200 mg/day or greater). Some have recommended using at least 50 mcg/day of ethinyl estradiol, but the effectiveness of this is unclear. Consider a nonhormonal form of contraception.
